Request Your Health Care Records
- You can request your health care records at any time.
- Please use our secure portal to submit your request. We will respond within 3 working days. If you no longer have your portal access then call 214.824.2009 and we can reset your access.
- You can also fax a letter requesting your records at 214.824.2081
- Add to your bookmarks for easier access.
